It has been nearly three weeks since the devastating loss of actor Cory Monteith, and fans are still trying to cope with the loss.

To honor their fallen star, Glee has released a touching memorial card through their Youtube channel on Thursday. Although the video is a only 12 seconds long, it encapsulates Monteith's one-of-a-kind spirit and positive energy that fans and friends remember.

Take a look at the short, but unbelievably sweet memorial video below.

Glee has chosen some of the most iconic Finn Hudson moments from the musical hit's first three seasons. From a football victory, to goofing around with the cast, and sharing a confetti-filled kiss with his on- and off-screen girlfriend Lea Michele, Glee certainly knows how to chose the scenes that tugs on our Monteith-loving strings. Recently Michele spoke out about her boyfriend's death, tweeting a happy photo of the couple and writing, "Cory Monteith will forever be in my heart."

Earlier this week, Fox's entertainment chairman Kevin Reilly revealed that the third episode in Glee's newest season will address Finn's absence from the rest of the series and will be connected with the late actor's troubled past.

"The third episode will write Finn out of the show. That episode will deal directly with the incidents involved with Cory's passing and the drug abuse in particular," Reilly said. However, Reilly wouldn't confirm whether or not Finn would die from drug overdose.

The episode will also feature PSA's starring Ryan Murphy and the Glee cast that will "speak directly with the audience" to bring awareness to those struggling with drug addiction. In a touching addition, all the proceeds from that episode's music will go directly to a fund in Monteith's name.

Reilly also revealed that he does not expect Glee will continue past a sixth season. "Look, never say never, but there's two very clear arcs to get to the end and conclude. If we discover a new crop of kids and there's some breakout in life, who knows? But right now we're just thinking about two more seasons," he said.

Murphy recently told us in an emotional conversation that he hopes Monteith will be remembered as "a champion of young artists and the underdog."

The season five premiere of Glee will air Thursday, Sept. 29 at 9 PM on Fox.
